Not-for-profit agency ACM Care (Autonomous Case Management, Inc.) began in 1999 after co-founder and Registered Nurse, Julie Hess, witnessed elderly, disabled and seriously ill patients in hospitals without the support of family. With no one to advocate for their care, they were often told by medical professionals that they had no choice but to be admitted to a nursing home. Hess knew other options existed and decided it was time to help.
ACM Care was founded on the belief that by using a team approach — experienced nurses and case managers — we could develop tailored solutions for clients. That would allow for better outcomes for clients who neither desired nor needed to move to a nursing home. ACM Care still uses that team approach today. We are a trusted partner for care solutions.
Experienced Medical Staff
With over 50 years of combined experience, our medical professional staff of registered and licensed practical nurses is highly qualified to oversee you or your loved one’s healthcare. Managers are certified in Case Management through the American Nursing Association and the Case Management Society of America. As members of the National Association of Professional Geriatric Care Managers, we have the knowledge families need. Our nurses average over 15 years of experience and hold active licenses in either Illinois or Missouri.
Experienced Geriatric Care Managers
With more than 60 years of combined experience, our Geriatric Care Managers hold Bachelors or Masters degrees in the social services or related fields. They must also attend over 24 hours of continuing education in care management each year. Our Geriatric Care Managers regularly participate in multidisciplinary teams outside our organization, further expanding our network of professional contacts.
